Scope:
Application for Setup of Medical Institutions in the Form of Sino-Foreign Medical Institutions
Documents Required (Printed on A4 Paper in the Following Order; All Materials must Bear a Common Seal.)
1. Opinions on setup issued by the county health bureau concerned:
Include preliminary opinions on projects raised in accordance with the plans of regional health and medical institution setup.
2. Application for Setup of Medical Institutions; Evaluation Form for Setup of Medical Institutions
3. The original letter of proposal or letter of intent for projects signed by legal representatives of both parties to medical institutions
Including project name, type, site, form of ownership, total investment, registered capital, size (beds and dental chairs), medical subjects of diagnosis and treatment, term of operation and proportions of rights and interests of both parties
4. The original of the Feasibility Study Report
5. Site Selection Plan
6. The original and copy of certificate of both parties to medical institutions①and notarial documents of a foreign party②
7. The original and copy of ID cards of legal representative of both parties to medical institutions①
8. The original of the bank reference letter and notarial documents of a foreign party②
9. Submissions relating to input of state-owned assets (including spot exchange)
(1) Written consent of assets owner to investment; (2) assets appraisal report; (3) written confirmation on proposed state-owned assets appraisal report issued by state-owned assets administration office; and (4) in the case of state-owned properties, certificate of title and plan of building layout.
10. Submissions relating to lease of state-owned assets
(1) Written consent of assets owner to leasing; (2) lease agreement or letter of intent; and (3) in the case of state-owned properties, certificate of title and plan of building layout
11. Other materials required by health authorities
